Our focus is on developing the skills necessary for individuals and families to maintain an independent and healthy lifestyle of their choice. We do this through supporting people to access services that reduce factors that contribute to disadvantage. We help people to LIVE LONGER, LIVE STRONGER

Aged and Disability Care

Larrakia Aged Care Services provides a comprehensive suite of support services to assist not only our First Australians, but all elderly to remain living independent and in their homes for as long they choose.

Through our Home Care Packages (HCP) and our basic Commonwealth Home Support Packages (CHSP), elders can access a variety of supports that offer flexibility and responsiveness to individual needs.

First Australians who are at least fifty years of age, who are frail and aged and others that are 65 years old, who experience difficulties in undertaking essential activities of daily living are eligible to receive these services.

Home Care Packages (HCP)

Larrakia Nation is an approved provider for the Department of Social Services Home Care Packages. These services include:

Domestic assistance
  • Household cleaning
  • Bed making
  • Dish washing
  • Assistance with laundry
  • General cleaning
Individual social support

Support to visit family and friends or just have a coffee or lunch at one of Darwin’s or Palmerston’s great coffee shops or places to eat.

Personal care

Assistance with eating, showering, grooming and medication prompting.

Social support groups

Held every Tuesday, Wednesday and Thursday. Activities include arts, craft, bingo, gardening and trips to interesting places around the Darwin and Palmerston region. Morning tea and lunch is provided.

Transport

Our friendly staff will support and transport you places you need to go. This may include grocery shopping, to the hospital, friends, doctors or specialist appointments. Priority is given to medical appointments.

Larrakia Nation is an approved provider for the Department of Health for Aged Care Services. These services include:

Social Groups 

Tuesday – 10am – 2pm

*Note: Our social groups do not operate on public holidays.

Our aged care clients are welcome to attend any one of our social groups, they are held at our Head Office, 76 Dick Ward Drive, Coconut Grove.

Social Groups engage in a variety of activities that are changing all the time. Some activities include:

  • Social outings such as picnics and BBQs around Darwin, fishing, art displays
  • Visits from school kids and other young people to spend time with our elders
  • Guest speakers, dance performances and entertainers
  • Arts and craft
  • Bingo
  • BBQs
  • Trivia

Morning tea, coffee and lunch are supplied.

We love to see any of our aged care clients attend our social groups. Come on in, have a cuppa and a chat.

Meals on Wheels 

Meals on Wheels is a community based service organised to help persons who have difficulty or are unable to prepare and cook meals for themselves.

From our kitchen we provide hot meals cooked and delivered fresh daily to your door.

We provide meals five days per week with the option of adding a weekend pack to tide you over the weekend.

At a low cost per meal you will be provided with healthy meal options of which examples include; a weekly roast lamb, beef or pork. Chicken curry, Thai beef noodles, lasagne and salad, quiche and salad and pork ribs and bok choi with rice.

Each meals come with a juice and fresh fruit daily and a desert twice per week.

Assistance with Care and Housing for the Aged

Our Assistance with Care and Housing for the Aged program (ACHA) is Commonwealth funded to offer short-term case management to assist elderly people link up with  services to address their housing and care related needs.

Services Include:

  • Comprehensive Assessment of needs and Action Planning
  • Advice & Advocacy on tenancy related matters
  • Locating suitable accommodation
  • Relocation assistance
  • Assistance to identify suitable community based support services e.g. Aged Care, Mental health etc
  • Referrals to health professionals

Care finders 

Care Finders offers to support older people who need extra, intensive support to access aged care services and other supports in the community.

Tenancy support program

Our Tenancy Support Program works within the local Darwin and Palmerston regions and provides support to the town camps and town camps within the broader community.

Our program offers practical and flexible assistance for residents to maintain their tenancies thereby minimising the risk of homelessness.

With a focus on physical, social and emotional well-being combined with advice and support in maintaining safe and health homes we assist people through:

  • Basic home repairs
  • Domestic assistance
  • Lawn maintenance
  • Financial counselling and advice
  • Accessing other community based support services
  • Rubbish removal • Advocacy and referral services
  • Support with difficult life circumstances such as domestic violence, unwanted visitors, mental health, substance misuse, disability, unemployment and homelessness.

Minbani Bebe Kinship Services

Minbani Bebe Kinship Care model is based on the Children Safe Family Together model designed by the Tangentyere Council.

This model acknowledges that Aboriginal children are best placed in their family homes, and that all effort should be made to prevent the removal of children.

Currently nine out of 10 children in out of home care in the NT are Indigenous. Often those children are placed in non-Indigenous families losing connection to kin, culture and country.

Our program aims to reunite children in care with the families and work with the family to design care that keeps children safe and connected.

Transport

Larrakia Nation provides transport assistance to all people accessing one of our other services.
